Further information for health professionals
Patients referred to our neurological programs receive a comprehensive, inter-disciplinary assessment in the physical, cognitive, emotional, functional, vocational and recreational areas. A rehabilitation plan is drawn up and regularly reviewed at team meetings. The involvement of the patients family is encouraged. Continuity of care is achieved through the provision of inpatient and outpatient programs and planned discharge that includes referrals to community services.
Return to work rehabilitation, driver assessment and training and follow-up review are available through The Victorian Rehabilitation Centre. Tools such as the Functional Independence Measure (FIM) and SF36 are used to measure the effectiveness of interventions.
