Pre-Admission Information
Your appointment time is for the arrival time at the hospital, NOT your procedure time. Please allow for a waiting period.
Administration staff will contact you regarding your health fund details. If someone else is likely to answer the phone and you do not wish him or her to know about your impending procedure, it is advisable to contact the Hospital immediately. Staff are not able to give details of procedures to anyone, but may identify themselves as being from a hospital therefore raising questions you may not wish to answer.
Please wear warm, comfortable clothing and low-heeled shoes.
It is preferable that this form, along with the Hospital Admission Form be completed and returned to Pine Rivers Private Hospital prior to your scheduled appointment where possible. If unable to do so, please bring these completed forms, along with your doctor’s referral to the hospital on the day of your admission. An adult family member or friend may witness your consent form signature at the end of this form. If you need a medical certificate or any other forms filled out by the doctor please inform the nurse when you are being admitted.
If you take any medications, please seek advise from your General Practitioner (G.P.) as to whether they can be taken before or after the procedure.
Pay careful attention to the Discharge Information at the end of this Information Letter to ensure all requirements are considered prior to your procedure. This will help ensure your safety following your procedure.
Your length of stay at the hospital will generally be between 1 ½ and 2 hours. On occasion, the Doctor may be running late, or a procedure may take longer than anticipated and this may cause your length of stay to vary.
