Post Hospital Care

Community Programs piloted a very exciting project, Post Hospital Care which was held over a 10 week period commencing mid April 2007. The 3 hospitals involved in the pilot were Knox Private Hospital, John Fawkner Private Hospital and Como Private Hospital.

The aim of the pilot was to formally integrate attendant care services into the hospital discharge-planning model as a viable option for patients requiring home support post discharge. This would provide a safe and smooth transition for Patients returning home.

Our trained Attendant Carers are able to provide assistance with Personal care, Domestic cleaning, Shopping, Meal preparation, Community access, Escorts to out-patient appointments, 24hr-care and Sleepovers.

In light of these significant benefits to the patient, we have expanded the Post Hospital Care service to other Healthscope hospitals. These include Dorset Rehabilitation Centre, The Victorian Rehabilitation Centre, Bellbird Private, Ivanhoe Private Rehabilitation Hospital and Ringwood Private.

This program is a much-needed service in the community and one which we are pleased to be a part of. Community Programs along with Post Hospital Care can be the ‘Stepping Stones to Independence’.
