Stepping Stones to Independence

Healthscope Community Programs aims to be a leader in accommodation, attendant care and support to individuals with acquired brain injuries, other disabilities and in post hospital care by providing ‘Stepping Stones to Independence’. Our vision is to provide flexible and individualised services with a focus on active participation and positive outcomes.

A Care Plan is developed for each person receiving services and outlines all program goals and objectives with the involvement of all relevant parties i.e. Case Manager, Client/Family. This plan is used as a daily working document for all our carers/support workers, to ensure the person’s individual needs are being met.
