Accommodation and Community Support


Healthscope Community Programs has 16 facilities, all of which have 24 hour care and staff support. Below, you will find more information on our facilities which have been grouped into the level of care our patients require.


1. High Care


Characteristics of residents – Very high personal care needs, high medical needs, require direct assistance with most activities of daily living, incontinent bladder and/or bowel, high communication impairment, poor cognition, safety issues, can require assistance with eating and transfers/mobility, siezure activity, significant challenging behaviours.

The following Community Houses are characterised as High Care:

 
2. Moderate Care


Characteristics of residents – Require moderate direct assistance / supervision and prompting with Activities of Daily Living, supervision to commence and keep on task, support from carers for daily tasks (at times with transfers and personal care), assistance with daily planning and structure due to cognitive impairment, moderate to mild behavioural issues.

The following Community Houses are characterised as Moderate Care:


3. Low Care


Characteristics of residents – Have skill level to live in own facility, needs intermittent support to address cognitive deficits, is generally ambulant or able to transfer and mobilise independently (in wheelchair), requires regular support to maintain skill levels, support and/or prompting for structuring daily routines, can access supports when needed with some structure and/or prompting, may require support for community activities and behaviour, service provider acts as safety net.

The following Community Houses are characterised as Low Care:

  • Ivanhoe transitional units (2)
  • Blackburn transitional unit
  • Clayton transitional unit
